Schedule for this position will be Monday through Friday from 8 AM to 5 PM EST. No weekends or Holidays will be required!

Must be willing to travel 50-75% to Yale New Haven hospital.

Position Summary

The Transitional Care Nurse is an integral member of the Aetna Case Management Team. The Transitional Care Nurse (RN) will work closely with patients, health care providers and partners to facilitate timely post discharge support for patients transitioning from an acute care setting to home. If home is not immediately possible, the Transitional Care Nurse will work with the Aetna CM team to facilitate an admission to an approved post-acute facility. The primary function of the Transitional Care Nurse is to assess pre-identified patients at the bedside in acute care facilities for program fit and coordinate support. The nurse will:

  • Collaborate with patients, case managers, and providers face to face in an assigned hospital setting to assess patients and refer to appropriate post-discharge programs

  • Collaborate telephonically with enterprise partners to communicate/facilitate care transition plans/coordinate referrals to CVS Health/Aetna programs

  • Coordinate with assigned CM team to obtain authorization for admission to a post-acute facility as applicable if home is not immediately feasible after the acute hospital discharge

  • Participate in continuous quality and process improvement

  • Completion of regulatory, compliance and professional development courses to meet the licensure and training requirements

    Required Qualifications

  • Minimum of 3+ years of experience as a Registered Nurse

  • Minimum of 2+ years of adult acute care experience

  • Must have active, current and unrestricted RN licensure in the state of Connecticut

  • Must be able to travel 50-75% to Yale New Haven Hospital

  • Valid and current driver’s license and auto insurance

    Preferred Qualifications

  • EMR/Epic experience

  • CCM/CCMC/ACM Certifications

  • Case Management or Transitions of Care experience

  • Ability to work independently and meet deadlines, multi-task successfully, answer emails and phone calls in a timely manner

  • CPR certification

  • Proficient with MS Outlook, Word, PowerPoint, and Excel

  • Ability to effectively communicate well with care team members

  • Ability to sit or stand for extended periods during the workday

    Education

  • Associate's Degree or Nursing Diploma Required

  • Bachelor’s Degree Preferred

Pay Range

The typical pay range for this role is:

$30.39 - $65.66

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
